IMPORTANT INFORMATION – PLEASE READ COMPLETELY
Health Requirements: All dogs must have current distemper, parvo and rabies vaccinations, bordetella is recommended but not required. Puppies must have at least two sets of puppy shots. All dogs and puppies must have been in your home without illness for the two weeks prior to class. Copies of vaccination receipts or verbal confirmation from your veterinarian will be accepted.
Missed Classes: There are no make-up classes if you miss a regularly scheduled class. With instructor’s permission, you are welcome to drop in at another Session, for a similar class, if available.
Withdrawals / Refunds: If you must withdraw from class, a full refund will be issued to you by Marie Devaney, dba The Fundamental Dog, if such withdrawal and request for refund is made directly to Marie Devaney no less than 48 hours before the start of Session. “No-shows” without notice will not be refunded. After the Session has started, no refunds will be given. In special situations, tuition may be transferred to another class at the discretion of The Fundamental Dog.
Training Services Provided: I understand that Marie Devaney, dba The Fundamental Dog (Trainer), will work directly with me and my pet to impart contemporary animal behavior knowledge that best fits our needs, and that successful companion pet programs depend on a combination of learned skills on the part of the pet and the owner. Trainer will make every reasonable effort to help us attain goals, but makes no guarantees of performance on the part of Owner or pet as a result of providing professional animal behavior consultation. Trainer, at her discretion, may refuse entry to a pet which is not healthy, is aggressive, or does not seem suitable for the services provided.
Waiver, Assumption of Risk, and Agreement to Hold Harmless: I understand that participation in dog training is not without risk to myself and my dog as well as any family members or guests who might attend. I hereby waive and release Marie Devaney, her Agents, The Fundamental Dog, and the owner/lessor of the facility from any and all liability of any nature, for injury or damage suffered as a result of my participation in any class or private lesson, including any damage or injury resulting from the action of any person or dog, or for any accident occurring on the premises or its surroundings. I agree to accept full responsibility for my actions, for the actions of my dog, and the actions of my family or guests.
Payment: Payment in full is required to reserve your space in class. There will be a $25 fee for any returned checks.
